Volunteer and Program Commitment Agreement

* I understand volunteers must be at least 15 years of age to be considered for the Children's National Volunteer Program. Depending on the assignment, the requirement may be 18 years of age.

* I agree to complete a background check, submit medical forms and vaccination proof, and fulfill other requirements as specified by the Volunteer Service Department before beginning volunteer services.

* I verify that I read the program description, requirements, and physical demands of the Children's National Volunteer Program and understand the role of a volunteer at Children's National.

* I understand completing this application is the first step of the onboarding process to join the Children's National Volunteer Program.

* I understand that I will be asked to present written, valid, official government documentation that I am legally present in the United States for the duration of the time that I will be volunteering at Children's National (i.e. VISA, passport, or government-issued photo ID).

* If accepted into the program I agree that I will complete all required online and in-person trainings as scheduled.

* If accepted into the program I agree that I will consistently volunteer during my assigned shift throughout the duration of the Children's National Volunteer Program.
